Poverty and Crime Linked to Differences in Newborns’ Brains

Summary: Infants born to mothers living in poverty have smaller volumes of gray and white matter across the entire brain. Additionally, babies born to mothers who live in high crime areas showed differences in brain activity to those whose mothers lived in safer areas. Those born to mothers who experienced crime had weaker neural connections between brain areas that control and process emotions. Maternal stress, researchers say, could be a main factor in the differences in neuroanatomy and brain connectivity.

Poverty and crime can have devastating effects on a child’s health. But a new study from researchers at Washington University School of Medicine in St. Louis suggests that some environmental factors influence the structure and function of young brains even before babies make their entrances into the world.

A study published online April 12 in the journal JAMA Network Open found that MRI scans performed on healthy newborns while they slept indicated that babies of mothers facing social disadvantages such as poverty tended to be born with smaller brains than babies whose mothers had higher household incomes.

MRI scans of full-term newborns born to mothers living in poverty revealed smaller volumes across the entire brain — including the cortical gray matter, subcortical gray matter and white matter — than found in the brains of babies whose mothers had higher household incomes. 

The brain scans, which were conducted only a few days to weeks after birth, also showed evidence of less folding of the brain among infants born to mothers living in poverty. Fewer and shallower folds typically signify brain immaturity. The healthy human brain folds as it grows and develops, providing the cerebral cortex with a larger functional surface area.

A second study of data from the same sample of 399 mothers and their babies — this one published online April 12 in the journal Biological Psychiatry — reports that pregnant mothers from neighborhoods with high crime rates gave birth to infants whose brains functioned differently during their first weeks of life than babies born to mothers living in safer neighborhoods. 

Functional MRI scans of babies whose mothers were exposed to crime displayed weaker connections between brain structures that process emotions and structures that help regulate and control those emotions. Maternal stress is believed to be one of the reasons for the weaker connections in the babies’ brains.

“These studies demonstrate that a mother’s experiences during pregnancy can have a major impact on her infant’s brain development,” said Christopher D. Smyser, MD, one of the principal investigators. 

“Like that old song about how the ‘knee bone is connected to the shin bone,’ there’s a saying about the brain that ‘areas that fire together wire together.’ We’re analyzing how brain regions develop and form early functional networks because how those structures develop and work together may have a major impact on long-term development and behavior.”

Babies in the study were born from 2017 through 2020, before the start of the COVID-19 pandemic. Smyser, a professor of neurology, of pediatrics and of radiology, said that to successfully scan newborns during the first few weeks of life, babies are fed when they arrive for scans because they tend to fall asleep after eating. They are then snuggly swaddled into blankets and a device that helps keep them comfortable and still. The brain scans take place while they sleep.

In the study involving the effects of poverty, the researchers focused on 280 mothers and their newborns. First author Regina L. Triplett, MD, a postdoctoral fellow in neurology, had expected to find that maternal poverty — referred to in the paper as social disadvantage — could affect the babies’ developing brains. But she also expected to see effects from psychosocial stress, which includes measures of adverse life experiences as well as measures of stress and depression.

“Social disadvantage affected the brain across many of its structures, but there were not significant effects that were related to psychosocial stress,” Triplett said. 

“Our concern is that as babies begin life with these smaller brain structures, their brains may not develop in as healthy a way as the brains of babies whose mothers lived in higher income households.”

In the second study, which implicated living in high-crime neighborhoods as a factor in weaker functional connections in the brains of newborns, first author Rebecca G. Brady, a graduate student in the university’s Medical Scientist Training Program, found that unlike the effects of poverty, the effects of exposure to crime were focused on particular areas of the babies’ brains.

“Instead of a brain-wide effect, living in a high-crime area during pregnancy seems to have more specific effects on the emotion-processing regions of babies’ brains,” Brady said. 

“We found that this weakening of the functional connections between emotion-processing structures in the babies’ brains was very robust when we controlled for other types of adversity, such as poverty. It appears that stresses linked to crime had more specific effects on brain function.”

Reducing poverty and lowering crime rates are well-established goals of public policy and public health. And the researchers believe protecting expectant mothers from crime and helping them out of poverty will do more than improve brain growth and connections in their babies. But if social programs that aim to help people reach their full potential are to succeed, the researchers said the policies must focus on assisting people even before they are born.

“Several research projects around the country are providing money for living expenses to pregnant mothers now, and some cities have determined that raising pregnant mothers out of poverty is good public policy,” Smyser said. “The evidence we’re gathering from these studies certainly would support that idea.”

Abstract

Association of prenatal exposure to early life adversity with neonatal brain volumes at birth

Importance  

Exposure to early-life adversity alters the structural development of key brain regions underlying neurodevelopmental impairments. The association between prenatal exposure to adversity and brain structure at birth remains poorly understood.

Objective  

To examine whether prenatal exposure to maternal social disadvantage and psychosocial stress is associated with neonatal global and regional brain volumes and cortical folding.

Design, Setting, and Participants  

This prospective, longitudinal cohort study included 399 mother-infant dyads of sociodemographically diverse mothers recruited in the first or early second trimester of pregnancy and their infants, who underwent brain magnetic resonance imaging in the first weeks of life. Mothers were recruited from local obstetric clinics in St Louis, Missouri from September 1, 2017, to February 28, 2020.

Exposures  

Maternal social disadvantage and psychosocial stress in pregnancy.

Main Outcomes and Measures  

Confirmatory factor analyses were used to create latent constructs of maternal social disadvantage (income-to-needs ratio, Area Deprivation Index, Healthy Eating Index, educational level, and insurance status) and psychosocial stress (Perceived Stress Scale, Edinburgh Postnatal Depression Scale, Everyday Discrimination Scale, and Stress and Adversity Inventory). Neonatal cortical and subcortical gray matter, white matter, cerebellum, hippocampus, and amygdala volumes were generated using semiautomated, age-specific, segmentation pipelines.

Results  

A total of 280 mothers (mean [SD] age, 29.1 years; 170 Black or African American, 100 White, and 10 other race or ethnicity) and their healthy, term-born infants (149 male; mean [SD] infant gestational age, 38.6 weeks) were included in the analysis. 

After covariate adjustment and multiple comparisons correction, greater social disadvantage was associated with reduced cortical gray matter (unstandardized β = −2.0; 95% CI, −3.5 to −0.5; P = .01), subcortical gray matter (unstandardized β = −0.4; 95% CI, −0.7 to −0.2; P = .003), and white matter (unstandardized β = −5.5; 95% CI, −7.8 to −3.3; P < .001) volumes and cortical folding (unstandardized β = −0.03; 95% CI, −0.04 to −0.01; P < .001). Psychosocial stress showed no association with brain metrics. 

Although social disadvantage accounted for an additional 2.3% of the variance of the left hippocampus (unstandardized β = −0.03; 95% CI, −0.05 to −0.01), 2.3% of the right hippocampus (unstandardized β = −0.03; 95% CI, −0.05 to −0.01), 3.1% of the left amygdala (unstandardized β = −0.02; 95% CI, −0.03 to −0.01), and 2.9% of the right amygdala (unstandardized β = −0.02; 95% CI, −0.03 to −0.01), no regional effects were found after accounting for total brain volume.

Conclusions and Relevance  

In this baseline assessment of an ongoing cohort study, prenatal social disadvantage was associated with global reductions in brain volumes and cortical folding at birth. No regional specificity for the hippocampus or amygdala was detected. Results highlight that associations between poverty and brain development begin in utero and are evident early in life. These findings emphasize that preventive interventions that support fetal brain development should address parental socioeconomic hardships.


Abstract

The effects of prenatal exposure to neighborhood crime on neonatal functional connectivity

Background

Maternal exposure to adversity during pregnancy has been found to affect infant brain development; however, the specific effect of prenatal crime exposure on neonatal brain connectivity remains unclear. Based on existing research, we hypothesized that living in a high-crime neighborhood during pregnancy would affect neonatal frontolimbic connectivity over and above other individual- and neighborhood-level adversity and that these associations would be mediated by maternal psychosocial stress.

Methods

Participants included 399 pregnant women, recruited as part of the eLABE (Early Life Adversity, Biological Embedding, and Risk for Developmental Precursors of Mental Disorders) study. In the neonatal period, 319 healthy, nonsedated infants were scanned using resting-state functional magnetic resonance imaging (repetition time = 800 ms; echo time = 37 ms; voxel size = 2.0 × 2.0 × 2.0 mm3; multiband = 8) on a Prisma 3T scanner and had at least 10 minutes of high-quality data. Crime data at the block group level were obtained from Applied Geographic Solution. Linear regressions and mediation models tested associations between crime, frontolimbic connectivity, and psychosocial stress.

Results

Living in a neighborhood with high property crime during pregnancy was related to weaker neonatal functional connectivity between the thalamus–anterior default mode network (aDMN) (β = −0.15, 95% CI = −0.25 to −0.04, p = .008). Similarly, high neighborhood violent crime was related to weaker functional connectivity between the thalamus-aDMN (β = −0.16, 95% CI = −0.29 to −0.04, p = .01) and amygdala-hippocampus (β = −0.16, 95% CI = −0.29 to −0.03, p = .02), controlling for other types of adversity. Psychosocial stress partially mediated relationships between the thalamus-aDMN and both violent and property crime.

Conclusions

These findings suggest that prenatal exposure to crime is associated with weaker neonatal limbic and frontal functional brain connections, providing another reason for targeted public policy interventions to reduce crime.
